Dear Mr. Herman:
Hartman & Associates, Inc. (HAl) is pleased to submit this proposal to provide professional land surveying services
for the Mill Creek Year 3 Improvement Program as an extension to our contract dated January 11, 1993, and
approved January 26, 1993. The scope of work for this project is as follows:
SCOPE OF WORK
l. Provide a topographic survey for the Mill Creek drainage improvements along the following routes:
A. The ditch across from Goldsboro Elementary School from 200 feet south of 20th Street to the
Goldsboro Elementary School property.
B. Survey the right-of-way of 20th Street from the ditch near the Goldsboro Elementary School
driveway.
C. Continue west along 20th Street extended to Mulberry Avenue then proceed north along Mulberry
Avenue to the southern area of the Year 2 Improvements.
2. Provide a topographic surqey for the land at the northeast comer of 20th Street extended and Mulberry
Avenue (approximately 3-5 Acres). Exact limits of this survey will be determined by Conklin Porter &
Holmes.
3. Import the survey information from the drawing provided by Don Holt, and verify inverts, pipe sizes, etc.
4. The survey will include a strip approximately 50 feet wide along the rome, as well as additional drainage
and sewer structures each side of the rotue. If drainage or sewer structures or pipes are within the route,
HAI will locate the nearest structures downstream and upstream to determine slope and direction of the
facilities.
5. HA1 will notify utility companies at least 48 hours prior to conducting the survey such that underground
utilities may be located.
SCHEDULE
HAl anticipates beginning the project within one week of receiving a notice to proceed from the
City and completing the project including submitting deliverables within 30 working days
thereafter.

COMPENSATION
Based on the work described above, we propose to provide the professional services on an hourly basis according to
the attached fee schedule with a not-to-exceed fee of $9,500.00.
