Dear Mr. MCCauley:
This letter is to serve as a renewal of the current contract for
the Employee Assistance Program (EAP) between City of Sanford,
Florida and P.P.C., Inc., dba Personal Performance Consultants,
Inc. which is effective for the period of October 1, 1990 through
September 30, 1991.
The new contract term will be from October 1, 1991 through
September 30, 1992. Based on a current population of 392, the
annualized professional fee will be $10,155.00, payable in four (4)
equal installments of $2,538.75 due on the first day of each
contract quarter. All remaining terms of the Agreement remain
~nchanged and in effect.
If this letter reflects your understanding of the new terms of our
~greement, please have each letter signed and witnessed by the
appropriate person and return one to Julia Harrison at P.P.C.,
