IRREVOCABLE DESIGNATION
I, ROY E. WILLIAMS, JR., hereby irrevocably direct
the Trustee of the Sanford Police Officer's Retirement System
to deduct the sum of $705.00 monthly from my Police Retirement
Pay and remit same to the Clerk of the Circuit Court, Seminole
County, Florida, Post Office Drawer 819, Sanford, Florida,
32772-0819.
This allotment is pursuant to the attached Court
Order in Seminole Circuit Court Case No. 88-712-CA-04-Z, and
shall be irrevocable without a subsequent Court Order authorizing
a modification or termination thereof.
Further, I irrevocably designate my former spouse,
DOROTHY M. WILLIAMS, as my sole survivor for any and all survivor's
benefits under the City of Sanford Police Officer's Retirement
System upon my death, pursuant to the attached Order.

FINAL,JUDGMENT.OFDISSOLUTION.MARRIAGE
THIS CAUSE was heard before the Court on Friday, March 25,
1988, On the Wife's Petition. From the evidence presented, the
Court finds that:
1. The Wife has been a resident of the State of Florida for
more than six months prior to filing the Petition.
2. Neither the Husband nor Wife are persons in the military
service as defined by the Amended Soldier's and Sailors' Civil
Relief Act of 1940.
3. There are no minor children of this marriage, none have
been adopted and the parties do not have custody of any minor
children at this time.
4. The Settlement. Agreement is fair and equitable and is in
the best interests of the parties.
5. The marriage is irretrievably broken.
6. The Wife is a "spouse or former spouse" as that term is
defined in 10 USC Section 1408 and was married to the Husband,
IROY E. WILLIAMS, JR., for 18 years during which he performed
creditable service in the U.S. Navy for retirement pay.
It is now therefore ORDERED AND ADJUDGED:
1. The legal bond of marriage between Husband and Wife is
][hereby dissolved.
2. The Husband shall pay the sum of $700.00 per month as
][permanent periodic alimony until or unless the Wife dies or
remarries.
Said $700.00 shall be paid by the Husband irrevocably
directing the City of Sanford to deduct said amount from his
~anford retirement check plus the statutory fee of $5.00 and pay
same to the Clerk of the Circuit Court.
Said payments shall be made payable to the Clerk of the
Circuit Court, Seminole County, Florida, Room 206, Seminole
County Courthouse, Sanford, Florida (mailing address: Post
Office Drawer 819, Sanford, Florida 32772-0819, together with the
statutory service charge of 3% of the payment or $5.00, whichever
~is less. Payments shall be made in the form 6f ~ash, 'caShier's
or and obligee for proper identification.
3. The Husband shall irrevocably designate the Wife,
DOROTHY M. WILLIAMS, as his sole survivor for the purpose of
receiving survivor's benefit under the City of Sanford Police
Officer's Retirement System upon the Husband's death.
4. The parties' Settlement Agreement is hereby approved,
ratified and confirmed in whole and is incorporated and made a
!ipart of this judgment of dissolution of marriage.
5. The Court retains jurisdictionn for any support
'!)obligations and unexpected property divisions of this action and
for the enforcement or modification of this judgment.
DONE AND ORDERED in Chambers at Sanford, Seminole County,
