MODIFICATION OF AGREEMENT
BETWEEN
FLORIDA DEPARTMENT OF COMMUNITY AFFAIRS

Cit~ of Sanford
This Modification is made and entered into by and between the State of Florida,
Department of Community Affairs, ("the Department"), and CRV of Sanfo~;d ("the Recipient")
to modify DCA Contract Number 03AL-2Y-06-69-02-029
WHEREAS, the Department and thc Recipient have entered into the Agreement, pursuant
to which the Department has provided a grant of $101,553 to Recipient; and
NOW, THEREFORE, in consideration of the mutual promises of the parties contained
herein, the parties agree as follows:
1. Clause (16)(a) Funding/Consideration is hereby modified to read as follows:
"This is a performance based cost-reimbursement Agreement. The Recipient shall
be reimbursed for costs incurred in the satisfactory performance of work
hereunder in an amount not to exceed a total of $114,214.40 subject to the
availability of funds. The total of funds shall be subdivided and reimbursement of
costs shall be limited to the amount indicated below for each Program and its
attendant Control Reference Number (CRN) for the indicated time frame."
Weatherization Assistance Program: CRN 03AL-2Y-06-69-02-029 Amount $16,462
(These funds are available September 29, 2002 through March 31, 2003)
This amount of funds are de-obligated from the program allocation. ( $3.081.60
Weatherization Assistance Program: CRN 03AL-3X-06-69-02-029 Amount $15,743
(These funds are available from the date this amendment is signed by both parties through
September 29, 2003)

Energy Assistance Program: CRN 03AL-2J-06-69-02-029
(September 30, 2002 through September 29, 2003)
Amount $45,387
Low-Income Emergency Home
Repair Program: CRN 03AL-35-06-69-02-029
(September 30, 2002 through September 29, 2003)

Performance of work for each of the above listed programs must be in accordance with the
provisions of Attachment A-2, Scope of Work. The amount expended in each county shall
approximate as closely as possible the amounts listed for each county in Attachment H.

Attachment H, County Allgqa~;ign~ is hereby modified to reflect additional
contractual funds, which are attached hereto and incorporated herein by reference.
All provisions of the Agreement being modified and any attachments thereto in
conflict with this Modification shall be and are hereby changed to conform with
this Modification, effective as of the date of the last execution of this
Modification by both parties.
All provisions not in conflict with th/s Modification remain in full force and
effect, and are to be performed at the level specified in the Agreement.
IN WITNESS WHEREOF, the parties hereto have executed this document as of the dates
set forth herein.
