Execution of the Work Order by CITY shall serve as authorization for the CONSULTANT to
provide for the above project, professional services as set out in the Scope of Services attached as Exhibit
"A," to that certain Agreement of Lot Split of Parcel 25-19-30-300-0040-0000 between the CITY and the
CONSULTANT and further delineated in the specifications, conditions and requirements stated in the
following listed documents which are attached hereto and made a part hereof.

SPECIAL CONDITIONS
The CONSULTANT shall provide said services pursuant to this Work Order, its attachments and
the above-referenced Agreement, which is incorporated herein by reference as if it had been set out in its
entirety. Whenever the Work Order conflicts with said Agreement, the Agreement shall prevail.
TIME FOR COMPLETION: The work authorized by this Work Order shall be commenced upon
issuance of a Notice to Proceed by CITY and shall be completed within two weeks (14) calendar days.
METHOD OF COMPENSATION:
(a) This Work Order is issued on a:

[ ] TIME BASIS METHOD WITH A NOT-TO-EXCEED AMOUNT
[ ] TIME BASIS METHOD WITH A LIMITATION OF FUNDS AMOUNT
(b) If the compensation is based on a "Fixed Fee Basis," then the CONSULTANT shall
perform all work required by this Work Order for the sum of ONE THOUSAND TWO HUNDRED
FIFTY SEVEN DOLLARS ($1,257.00). In no event shall the CONSULTANT be paid more than the
Fixed Fee Amount.
(c) If the compensation is based on a "Time Basis Method" with a Not-to-Exceed Amount,
then the CONSULTANT shall perform all work required by this Work Order for a sum not exceeding
DOLLARS ($ ). The CONSULTANT'S compensation
shall be based on the actual work required by this Work Order.
(d) If the compensation is based on a "Time Basis Method" with a Limitation of Funds
Amount, then the CONSULTANT is not authorized to exceed the Limitation of Funds amount of
DOLLARS ($ ) without prior written
approval of the CITY. Such approval, if given by the CITY, shall indicate a new Limitation of Funds
amount. The CONSULTANT shall advise the CITY whenever the CONSULTANT has incurred
expenses on this Work Order that equals or exceeds eighty percent (80%) of the Limitation of Funds
amount. The CITY shall compensate the CONSULTANT for the actual work performed under this Work
Order.
Payment to the Consultant shall be made by the CITY in strict accordance with the payment terms
of the above-referenced Agreement.
It is expressly understood by the CONSULTANT that this Work Order, until executed by the
CITY, does not authorize the performance of any services by the CONSULTANT and that the CITY,
prior to its execution of the Work Order, reserves the right to authorize a party other than the
CONSULTANT to perform the services called for under this Work Order if it is determined that to do so
is in the best interest of the CITY.

provide professional land surveying services for the above referenced site.
Project Description and Location
The client desires a sketch and legal description performed for the purposes of property sale. The
project is located in the City of Sanford adjacent to the City Hall.
Scope of Services
As we understand your request WSM will prepare a sketch and legal of the site in accordance with
the minimum technical requirements. The limits will extend to the South R/W line of Seminole
Boulevard, the East R/W line of Myrtle Avenue, the North and East lines of tier 5, Lake View
Apartments, North R/W line of Fulton Street, and the line shown on survey by Benchmark Design
Services, Inc.
Client Responsibility
The client will provide complete Official Records Book copies of legal descriptions and Lakeview
Apartments plat for the property. Provide digital file of Benchmark Design Services Survey.
Provide access to the property and contact names prior to our beginning of work on the site.

Deliverables
All information gathered will result in a 8 ½ x 1 ldrawing at a scale of 1"=100 feet. All items
requested in the above noted sketch of description requirements will be shown or noted on the survey
drawings. All work will also be performed in accordance with the "Minimum Technical Standards
for Surveys" as defined in Chapter 61G17-06 of the Florida Administrative Code, pursuant to
Section 472.027, Florida Statutes.
Deliverable products will include six (6) signed and sealed paper copies.
Schedule
WSM is prepared to commence work within a week of receipt of a signed notice to proceed.
Delivery of our work will be within two (2) weeks, subject to review and revisions as necessary.
Compensation
WSM will provide you with the above described services and products for a total amount of One
Thousand Two Hundred Fifty Seven Dollars and No Cents ($1,257.00) based on the following
man hour breakdown and associated fees.

LICENSE AGREEMENT
This electronic media is the properly ofWeidener Sunteying and Mapping P.A. (the surveyor). It only licenses the use of
the electronic media and does not transfer ownership of this copy.
You may only:
a) Use this electronic media on the specific project it was prepared for;
b) Copy the elec~'onic media only as necessary to u.~ it; and
c) Transfer thc electronic media to another person only if that person agrees to accept the terms and conditions
of this license.
These are the only rights granted. Among other thin?, yOU may not: rent, lease, modify, l~tnslate, disassemble,
dccompile, reverse engineer, or cmat~ derivatives ba.~d on the electronic media. If you fail to comply with any terms of
this license agreement, YOUR LICENSE IS AUTOMATICALLY TERMINATED.
Rationale
The surveyors products are printed hard copy maps and/or written reports issued for the project, whereas electronic
media, including CADD flies, are tools for their preparation. The client's attention is directed to the Minimum Technical
Standards of the State of Florida, Chapter 61G 17-6, FAC, which govern. As a convenience to the client, the surveyor will
furnish to the client, or his desigaate, both hard copies and electronic media. Ia the event of a conflict in their content,
however, the printed hard copies bearing the signature and raised seal of the surveyor shall take precedence over the
electronic media.
Surveyor's electronic media are furnished without guarantee of compatibility with the clienfs software or hardware and
the surveyor's sole responsibility for the electronic media is to furnish a replacement for defective electronic media
within a thirty (30) day period following final delivery.
The surveyor retaim ownership of the printed hard copy maps, reports and electronic media. The client is graated a
license for their use. Being that the surveyor's work involves existing conditions and/or opinions ns to facts determined
by thc survcyor's observations, it is inappropriate for the client to make changes in any electronic media provided.
Accordingly, uae of the elec~'onic media for modification, extension or expanaion, unless under the direction of the
surveyor, shall be without lhbility to thc sm'vcyor and the surveyors consnltants, if any. With the above in mind, thc
electronic media is fu~i*hed in *read only~ form~ Any t~ .tapering with the electronic media of the surveyor
immediately and automatically terminates the license granted herein.
Because data stored in electronic media form caa be air. od, either intentionally or unintentionally, by transcription,
machine error, environmental factors, or by opemtor~, it is agreed that thc client and/or thc user of thc electronic media
shall idenmify, defend and hold harmless the surveyor, the surveyors con~ulUmts, and the officers and employees of any
of them from and against any and all cla/m~, liabilities, damages, losses and costs, including but not lira/ted to costs of
defense, arising out of changes or modifications to the data in elec/xodic media form in the possession of the chent and/or
the user, or released to others and for any use of the printed hard copy, reports and elec~onic media outside the license
granted hereuader.
General
BY USING THIS ELECTRONIC FILE YOU ACKNOWLEDGE THAT YOU HAVE READ THIS AGREEMENT,
UNDERSTAND IT AND AGREE TO BE BOUND BY ITS TERMS AND CONDITIONS. YOU FURTHER AGREE

GENERAL TERMS AND CONDITIONS
I) AIl surveys will meet thc Minimum Tcctmical Standards of the State of Florida in 61G 17-6, FAC, for the type of survey
specified. WSM does not warrant nor guarantee accuracy or completeness.
2) The scope ofwurk specified in the proposal includes only those services and deliverables enumerated therein.
3) In all boundary surveys and similar endeavors (fights of way or easement acquisition, for example), WSM recommends
that a full and complete title search be provided to us to make certain that all easements and fights of others are
disclosed. WSM does not search the public records as a matter of course.
4) Only visablc surface evidence ofutilities will be located (on thnse projects wbere same is incinded in the scope ofwork)
and WSM makes no representations as to the utility itself. Utility records may be consulted for convenience but WSM
does not regularly provide utility research. All utilities may not be shown. WSM may request a "mark out" but WSM
can not compel this service - if a "mark out" is actually pmvided WSM will locate the marks. WSM strongly
recommends that our clients satisfy themselves as to all utility matters.
5) WSM is prohibited by OSHA from entering "confined spaces" such as inlets and manholes. Accordingly, all
measurements of pipe sizes, inverts, etc. must be made from the outside and may not meet normal surveying standards.
If access to "confined spaces" is mandatoi'y for your project, WSM may provide a separate proposal for monitoring
instrtunents, ventilators, special safety equipment, and the like necessary for entry.
6) Maintenance may be needed to access inlets and/or manholes. WSM will not clean out such structures - we will,
however, report same to you for action. When aH suucturcs are clean, WSM will make one (1) additional attempt to
acquire measurements without charge.
7) Route surveys for improvements in dedicated rights of way do not include boundary surveys of the right of way lines
unless specifically stated. In such cases, the rights of way are delineated to national map accuracy standards only and
should not be considered sufficient for the acquisition p.f properVy or casements.

24 hours fullowiag staking for quality conU'ol - WSM will not accept responsibility if the staking is utilized by the
client before the expiration of this period. All construction stoking is done once - any restaking, regardless of the
cause of the loss, will be provided only at additional fee. Any and all claims being made agaiust WSM for
construction staking mast be in writing and prior to any remedial work so WSM has an opportunity to visit the
site and verify the validity of the claim.
9) Invoicing will be pwvided monthly and/or at substantial completion of the work Invoicing is due upon presentation.
WSM reservea ~be right to suspend work at any time that invoicing is in arrears more than 30 days. if lta'x~ sum,
invoicing will be made on the percent complete at the time. If hourly, WSM will sum all rifles necessary to the work at
specified rotes. 'Back up" data that is requested by the client will occasion additional charges of $2.00 per sheet,
minimUlll of $25.00.
10) Invoicing in an'ears more ~han 30 days will be assessed a late charge at the rate of 11/~ percent per month, compounded
monthly, from the date of invoice until full satisfaetiun.
1 I) Hourly billing of fees will include all direct charges for field work, office computations, supervision, drafting, quality
control, and the like that are necessary to the work, in our sole opinion. WSM "overhead" does not include such direct

14) In any dispute, the prevailing party shall be entitled to receive reasonable attorney fees, court costs and the like.
15) Any agreement to perfunn services shall be governed by the laws of the State of Florida.
16) If any provision of these terms and conditions is held to be invalid, void or unenforceable, the remaining provisiov..s
shall remain in full force and effect.
17) WSM does not represent that any governmental approvals can be or will be obtained. WSM will, however, exercise
best efforts in this regard should it be disclosed to us that approvals are desired.
